As Medical Director of Partners Connected Care, Dr. Moore led a cardiac telemonitoring program that reduced heart failure readmissions by more than 40 percent.
She was founding director of the Advanced Heart Failure and Transplant Fellowship at Massachusetts General Hospital, and has directed heart failure fellowship training at three institutions.
